I'm an individual or organization requesting a transcript on behalf of a physician. What do I need to know?

When submitting multiple transcript requests, you must create a separate account for each individual you are requesting transcripts for. Use the "Create an account" link on the initial log in page for each new account you need to set up.
You may designate up to two recipients for each $70 fee, regardless of the number of examinations (USMLE, FLEX, SPEX) included on the official transcript. For example, the cost for 1-2 recipients is $70, 3-4 recipients is $140, 5-6 recipients is $210, etc. Once a transcript request has been submitted, only the recipient(s) listed on the request will be processed; additional recipients cannot be added and the fee will not be applied to an additional recipient.
Most state boards receive transcripts electronically. For boards that do not receive transcripts electronically and for all other recipients (e.g., examinees, residency programs), transcripts are sent via first class mail unless Express shipping is requested. Please be advised that the U.S. Postal Service does NOT guarantee the timing of delivery for first class mail. We recommend you utilize the Express service option to guarantee delivery of your transcript in a timely manner. Express shipping is an additional $25 per recipient and is only available to addresses within the US and its territories. Express shipping does not deliver to P.O. Box addresses.
If I am using the Federation Credentials Verification Service (FCVS), do I need to request a transcript using this form?
keyboard_arrow_downIt depends on who your FCVS profile is being sent to. Transcripts are included for profiles sent to state licensing authorities only; the fee paid for FCVS services includes the price of the transcript(s). If you are having your profile sent to yourself or to a third-party (residency program, hospital, etc.), you will need to submit an official transcript request and have your transcript(s) sent separately.
When or why would I need to request a transcript?
keyboard_arrow_downUSMLE examinees who no longer have access to their score report. USMLE score reports are available for approximately 365 days from the date of your e-mail notification that it is available. After 365 days, the score report is removed from the website and your scores will be provided to you only in the form of an official USMLE transcript, which is available for a fee. The graphical performance profiles included on your original score report are not included on USMLE transcripts.
Applicants for initial licensure in most states will need to submit one.
Licensed physicians may need to submit one in order to obtain a license by endorsement from another state.
Students and/or physicians may need to submit one to third-parties such as residency programs or may want one for their personal records.
Can I send a transcript to myself?
keyboard_arrow_downYes. For FLEX and SPEX scores the FSMB can send a transcript to any recipient designated. For USMLE scores, FSMB can send transcripts to any recipient if the examinee has 1) completed all three Steps of the USMLE examination, or 2) completed Step 1, Step 2 CK and Step 2 CS and is already registered for Step 3. Please review the requirements specified on the USMLE website for full details.
Is my payment and my information secure?
keyboard_arrow_downWe use 128-bit encryption (SSL) for transmission of data. We never store the credit card information, except the last 4 digits.
Which credit cards do you accept?
keyboard_arrow_downWe only accept Visa, Mastercard or American Express.
What scores and information are provided on a transcript?
keyboard_arrow_downThe FSMB maintains scores for the FLEX, SPEX and USMLE Steps 1, 2CK, 2CS, and 3 examinations only. The official transcript includes: biographical information; a complete history of scores (including incomplete attempts) for the designated examination(s); information regarding findings of irregular behavior and actions reported to the FSMB; and an indication, if applicable, of any board action reported to the FSMB by a state board or other regulatory agency. The graphical performance profiles included on your original USMLE score report are not included on your USMLE transcript.

What is the maximum number of licensing authorities I can select on one online form?
keyboard_arrow_downUp to 20 recipients may be entered on one online transcript request. Additional recipients would require submission of another online form.
How can I cancel my request?
keyboard_arrow_downOnce a transcript request is submitted it cannot be canceled.
Can I add a second recipient at a later date?
keyboard_arrow_downNo. Additional recipients cannot be added once a transcript request is submitted. If you need to add additional recipients, you will need to complete and submit a new transcript request and appropriate fee.
Do international medical students and graduates request transcripts from FSMB or ECFMG?
keyboard_arrow_downFSMB sends all transcripts for FLEX and SPEX scores. For USMLE scores, please use the following chart to determine which entity you should contact:

How are my transcripts sent?
keyboard_arrow_downMost state medical boards receive the transcripts in an electronic format via a secured website. Electronic transcripts are posted daily to a secure website available to the medical board. All other recipients receive the transcript as a secure paper document via first class mail (unless Express shipping is requested).
How do I confirm that a licensing authority has received my transcript?
keyboard_arrow_downPlease contact the licensing authority to verify receipt of the transcript. Transcripts are mailed within five business days of receiving a complete transcript request. Some state medical boards have elected to receive transcripts electronically. Electronic transcripts are posted daily to a secure website available to the medical board.
Who do I contact for technical assistance?

Why can't I enter a transcript request at 3:00am on Sunday?
keyboard_arrow_downOur site is down every Sunday for routine system maintenance between the hours of 2:00 a.m. and 9:00 a.m. Central Time. If you receive an error outside of these hours, please record the message and report this to technical support at usmle@fsmb.org or call 817-868-4041. If possible, please recall the steps you took to get to this error, your browser type and version, and the date and specific time the error occurred.
