 UA requires the following forms:

NotaryCam is an electronic notary service that will securely and legally notarize these forms online, and send them directly to the state medical boards. (The forms are hosted on the NotaryCam site.) Additional fees apply, and are independent of FSMB/UA fees.

NOTE: Online notaries cannot use a birth certificate as a form of identification. To use this service you must present your passport.

Physicians applying for licensure at those UA accepting states, that are not listed here, should mail in the document to the appropriate state medical or osteopathic board contact provided in your UA state instructions. Please refer to the Traditional Notary Instructions for additional information.
